CONSENT FOR VIRTUAL PARTICIPATION

As the parent or legal guardian of the above-named participant,I hereby give my consent for virtual participation to the art and activity program organized by ROWAC. This program may involve videotaping of some sessions and virtual instructions on cutting cardboards as well as using glue, pencils and other writing instruments. I will accompany my child during the exercise. I hereby confirm to be the sole responsible person for my child's wellbeing throughout these sessions.
